Digital care hub shortlisted for national awards

An innovative team that delivers digital health and care support for people across Bradford District and Craven has been shortlisted for the prestigious Health Service Journal Awards.

The Digital Care Hub, based at Airedale NHS Foundation Trust, is a service that has been leading the way in technology-based and remote monitoring solutions that helps people stay well at home for longer and reduces the number of hospital admissions. People being cared for by the district-wide team are often some of the most vulnerable with a range of long-term health conditions.

The Digital Care Hub has made the shortlist for the Digitising Patient Services Initiative category.

The support provided by the Digital Care Hub involves close working with a range of health and care professionals, ranging from telemonitoring through to the recently launched MyCare24 service supporting people with chronic obstructive pulmonary disease.

In addition, its Goldline service provides support for people at end-of-life with an evaluation showing it reduced hospital admissions by 23%, enabling people to die on their own terms and in their preferred place. This means that hospital-deaths for Goldline patients are among the lowest in the country resulting in only 14% of people on Goldline dying in hospital compared to the national figure of 46%.

The entry to the Health Service Journal Awards demonstrates how the Digital Care Hub is a key part of the Act as One health and care partnership for Bradford District and Craven.

This is a real recognition of how we are working in partnership across our place so that we can scale up support for people across Bradford District and Craven.

“The Digital Care Hub has really come into its own during Covid from providing 24/7 remote monitoring support that offers assurance for people needing care, their family members as well health and care professionals to developing services that help people feel confident taking care of their own health without needing support from any services. This has been invaluable during a period when our services are experiencing unprecedented pressures.”
