A recent survey by Diabetes UK has revealed that large numbers of people with diabetes do not have a personalised care plan in place.
If you have diabetes, it’s really important that you get the information and support you need to manage your own condition and reduce the risk of serious complications.
Developing a care plan can help with this. Diabetes UK has good advice on care planning and managing your diabetes. It’s about being involved in decisions about how your diabetes is managed, and the care you receive.
To help you think about the care you need, the charity has also produced a handy checklist of 15 healthcare essentials. These are the basic checks and services that everyone with diabetes should receive.
