We are a team of four Heart Failure Nurse Specialists who manage and support patients with chronic heart failure (caused by left ventricular systolic dysfunction) and their carers and families in the community of Airedale, Wharfedale, Craven & Bentham. Our aim is to help patients achieve the best possible quality of life through all stages of this chronic life limiting illness, from diagnosis through to palliative care. We strive to ensure patients remain at home or their preferred place of care, to avoid hospital admission where possible.
The team have advanced clinical assessment skills to facilitate heart failure management, titrate, optimise and prescribe medications, educate, devise appropriate management plans and work alongside multi-disciplinary team members.
We are based at the hospital, work within the community and offer home visits and designated nurse led clinic appointments. The team are supported by the cardiologists, physicians, telehealth hub and in primary care by GPs, practice nurses, palliative care specialists, community nurse specialists, district nurses, matrons, ANPs, allied health professionals, social and voluntary services.
Referrals are accepted from consultants and GPs. Additionally we offer education and support to health care professionals across both primary and secondary care.
