I am the Intermediate Care Team Leader for the Craven Collaborative Care Team which is part of community services covering the Airedale, Wharfedale and Craven area providing clinical care and support to people in crisis within their own home and supporting discharges from hospital. The team also provides OOH District Nursing Service in the Craven area. I qualified as a registered nurse in Leeds 2011 and worked in acute elderly medicine at St James University Hospital in Leeds for a year before joining Airedale NHS Foundation Trust in 2012 where I worked as a Staff Nurse at Castleberg Community Hospital based in the Yorkshire Dales town of Settle for 5 years. Following closure of the hospital, I then began working with the collaborative care team based at AGH and became junior sister after 4 months. I transferred to the Craven team in 2020 and became acting Team Leader before progressing into a permanent position in 2021, I am based at Skipton Hospital.
