Craven District Nursing Team (Craven Virtual Ward)
Welcome
District Nurses are qualified Registered Nurses additionally trained to provide specialist nursing care and advice to adults in the Craven Community who are permanently or temporarily housebound
The District Nurse team
The District Nurse leads a team of Registered Nurses and Health Care Support Workers, who provide care and support to you and your family/carers. All staff carry a photo identity card.
The District Nurse will discuss your nursing needs with you and agree a plan of care. A record of this care will be on an electronic record and paper light version will be left in your home. The date and time of your visit will be arranged according to your needs. This can vary from day to day and whilst we endeavour to provide continuity, this cannot always be guaranteed

- Specialist nurses for continence, end of life care, diabetes, stoma care, heart failure and wound care
- GPs, Practice Nurses and Community Matrons

How to refer:
A referral can be sent via SystmOne or email: anhsft.craventeam@nhs.net . For any other NON-URGENT issues please call us on our direct Single Point of Access number 01756 701754. For any URGENT calls only during office and out of hours please bleep us via Airedale switchboard.
