Our Community Neurological Specialist Nursing team consists of

The service includes Neurology, Multiple Sclerosis (MS) and Parkinson’s Disease (PD) Specialist Nurses covering Airedale, Wharfedale, Craven (Neurology and MS only).

The service provides advice, education and support for people with a diagnosed neurological condition as well as a responsive and accessible service to people in their own home, residential and nursing homes and also in community nurse led clinics.

We perform advanced nursing clinical assessments to provide individualised management/care plans involving the wider multi-disciplinary team as appropriate, promoting self-care.

We work alongside Neurologists, Rehabilitation Consultants and other relevant consultants to provide advice, education and support to colleagues within the Trust and Primary Care.

We offer advice and support from diagnosis, throughout the progression of their condition to end of life and advise on symptom management, encompassing physical, emotional, psychological and spiritual needs.
