Digital Care Hub

Digital Care Hub

The Hub currently provides round-the-clock care support to hundreds of care homes nationwide, more than 30 prisons, and thousands of patients with long-term conditions and patients in their last year of life, living in their own homes.

The hub is now providing clinical support to all care homes in Bradford District and Craven Place. The service is enhanced by support from GPs, hospital consultants, mental health specialists, community and social care.
