MyCare24
Welcome
MyCare24 is a dedicated 24/7 telephone and video service to support people to manage their long-term condition at home.
People on this service are supported to understand and manage their long-term condition proactively at home.
This service can stop people going into hospital or help them go home sooner with extra support. This can include providing long term remote monitoring, clinical support, and advice as well as short-term intensive support for people who would otherwise be in hospital.
The MyCare24 team is based at Airedale Hospital and work closely with the emergency department (A&E), your GP, specialist nursing teams and other local health and care services.

When to use the service
People supported by MyCare24 are advised to call when they are:
• feeling worried or unwell
• finding it difficult to get help during the day or at night
• not sure what to do
• thinking of coming to the emergency department (A&E) at the hospital
• struggling to contact their GP, specialist nurse or other service
Calls are answered by one of our experienced healthcare professionals.
What happens when you call
The MyCare24 team will assess the patient’s condition, give advice, support and contact other services on their behalf, if this is needed. This may avoid the need to come into hospital, but we can arrange this when necessary.
The MyCare24 team can support people over the telephone or send a video link (via email or text message) to enable people to see the health professional face to face.
Accessing your health record
With permission, the MyCare24 team will access a patient’s health record to see information about their condition and medications. The details of the call will be entered into the health record so that GP’s and other services providing care are kept informed and up to date.

NGT lite app for Deaf, hard of hearing and speech impaired patients
To support patients who may be Deaf, hard of hearing or speech impaired the app, NGT Lite, is available to download onto your smartphone or computer device.
