Patient initiated follow up (PIFU)

Patient Initiated Follow Up (PIFU) Information:

As discussed, and agreed with your clinician at your last appointment, you have now been discharged with a PIFU.

PIFU puts you in control – this means that instead of automatically having a follow up appointment booked at a specific time for the future, you have the flexibility to contact us for an appointment if you feel that you need to, within the agreed timeframe.

If you feel that you do not need a follow up appointment, that is fine – you do not need to contact the service. Patient initiated follow up is designed to be more convenient for you, saving you time and travel.

How long am I on Patient Initiated Follow Up?

You can contact the service directly without needing to see your doctor first, within the agreed time scale with your clinician.

What happens after my Patient Initiated Follow Up time ends?

After your Patient Initiated Follow Up time has ended, you will be discharged and therefore will need to contact your GP if you experience any further issues related to your condition


Once the PIFU time has expired you may receive a text from Airedale NHS Foundation Trust requesting feedback for our services.  We are always grateful for any feedback you provide.

Concerns about your care

If you have any concerns or questions about your care, we advise you to talk with the service or the department manager in the first instance as they are best placed to answer any questions or resolve concerns quickly. A PIFU Discharge letter has been sent to your GP so they are also informed. You can view this letter in your NHS records/App.




Which services provide PIFU?

Physiotherapy - MSK and Rheumatology

If you are on a PIFU with the Physiotherapy – MSK and Rheumatology outpatient service you can find their contact details by following this link please note the telephone is monitored between Monday to Friday between 09:00 and 16:00 by our reception team. If there is no answer, please leave a voicemail, and someone will call you back (however this may not be the same day). If leaving a voicemail, please state:

  • Full name
  • Telephone number
  • Date of Birth
  • That you are on PIFU list

The reception team will be able to book you a follow-up appointment with a clinician.

Ideally this appointment would be with the clinician who initially treated you – however, in some circumstances it may be with an alternative. Please note the appointment may be a telephone call in the first instance.
